They send you a physical box with the activation code and some explanatory materials inside.  Actually, I just ordered last night and it came early this afternoon.  Pretty good service.

just want to make sure that the Buy.com iPod Applecare extends the warrranty to 3 years from the original purchase date.

The Apple homepage offers the same plan, but only extends the warranty 1 extra year. (2 years from the original purchase date)
